Medical Details*

I acknowledge that fees are non-refundable and non-transferable. I also understand that SHMA has the right to cancel 1 student class per term without refund. Students may catch up a cancelled SHMA class within 9 months of the cancellation as per our Tc's and C's*

I understand that parents must check the SHMA website for all student information (Term classes, Kit requirements, Snacks allowed, late fees etc), and if they need to contact SHMA, they should use the Contact Us form on the website. *

I recognize that our contact number (07714 425883) is intended solely for notifying us about changes in student collection or if a student is missing class.*

I know that I can absolutely reach out to SHMA if the information isn’t available on the website or if I need to notify the class instructor about a personal matter regarding my child (such as an injury).*
